Job Description
POSITION OVERVIEW
The St. Charles CARE Center is a family assessment center, providing screening and assessment services to young people and their families who are experiencing challenges at home, school, and in the community. Using our network of organizations across and beyond the parish, we provide families with meaningful connections to support and break down barriers to accessing services.
Over 500 young people and their families experiencing challenges, like involvement in the justice system, have been served by the CARE Center since 2018. During this time, we’ve expanded our partnerships beyond the justice system, working closely with the school district to proactively support children and families. With the help of our Program Coordinators (Navigators), young people and their families work to identify their strengths, set goals, and take meaningful steps toward lifelong success.
Reporting directly to the Managing Director, the Program Coordinator is responsible for the high-quality implementation of our program model, Family SBIRT (Screening, Brief Intervention, and Referral To...). The Program Coordinator will work directly with participating families to identify areas of need, develop a comprehensive service plan, connect them with appropriate social services and resources, and offer ongoing support and service navigation. Additionally, the Program Coordinator will facilitate restorative justice practices, including restorative circles, with young people and other members of the community when appropriate.
The majority of participating families are referred from the 29th Judicial District Court in St. Charles Parish, the St. Charles Parish Sheriff’s Office, or the St. Charles Parish Public Schools. Through these referral partnerships, the Program Coordinator will work closely with various court and education professionals including court administrators, law enforcement officers, principals, student advocates, and school counselors.

RESPONSIBILITIES
- Conduct formal intake and screening process with participating families
- Conduct brief intervention sessions with participating families using motivational interviewing techniques
- Identify participants’ needs for additional outside services
- Establish a service plan in partnership with each family (e.g. determine follow-up session schedules, determine service accessibility, including transportation, etc.)
- Plan and facilitate restorative circles with young people and other members of the communities (when applicable)
- Communicate with families regularly as guided by their individualized intervention plan; includes phone calls, text messages, and home visits
- Provide detailed reports to referral partners on participants’ progress (as applicable)
- Complete all required program paperwork with participating families
- Maintain participant paper and electronic records by logging detailed notes on progress and events
- Collaborate regularly with an interdisciplinary team of professionals including court personnel, law enforcement officers, treatment providers, and school district personnel to review participant progress and identify areas where families may need additional support
- Implement program evaluation strategy, which includes data collection, management, and analysis
- Prepare program-wide as well as participant reports by collecting, analyzing, and summarizing treatment and program data
- Enhance professional knowledge by participating in educational and training opportunities, reading professional publications, and maintaining and growing professional networks

QUALIFICATIONS
- At least 2 years of experience working with underserved populations (professional and/or volunteer experience)
- At least a Bachelor’s degree in a related field such as psychology, social work, criminal justice, or sociology
- A reliable vehicle
SALARY & BENEFITS
The starting salary ranges from $38,000 to $40,000 annually based on qualifications. Opportunity for promotion to Senior Program Coordinator after three years of service. Senior Program Coordinators have a maximum earning potential of $50,000. Full-time employees are eligible to receive benefits including
- Medical, dental, and vision insurance (75% of premiums covered by the employer)
- 15 paid holidays annually
- 10 days of Paid Time Off during first year of employment
- Friday remote work days

We aim to identify and address the mental and behavioral health needs of children and families in St. Charles Parish through evidence-based screenings, restorative interventions, and referrals to services. The CARE Center works collaboratively with a network of organizations and providers across and beyond the parish, maintaining strong partnerships with the court, school system, and community-based organizations.
