Case Manager (QIDP) – Empowering Adults with Developmental Disabilities!

We're looking for a compassionate, organized, and dedicated Case Manager to help guide and support our residential clients—adults with intellectual and developmental disabilities—on their journey to independence and well-being.
About the Role
As a Case Manager at Magnolia, you will play a pivotal role in ensuring the growth and success of our clients. You will be part of a dynamic team responsible for crafting and executing individualized service plans (ISPs), managing daily living activities, and ensuring the group homes run smoothly and efficiently. You’ll also provide functional supervision to Direct Support Professionals (DSPs) and contribute to creating a supportive, person-centered environment for our clients.
Key Responsibilities
• Collaborate with team to develop and implement annual ISPs, focusing on client goals that foster independence, self-sufficiency, and enhanced quality of life.
• Uphold the dignity and respect of all individuals with disabilities and their families, using person-centered planning for goal development and service delivery.
• Create a home-like environment for clients, incorporating positive behavior management strategies and training.
• Conduct regular group home inspections to ensure safety, cleanliness, and adequate supplies while maintaining compliance with dietary and quality-of-life standards.
• Functionally supervise DSPs in their daily responsibilities, ensuring the highest quality of care and support for our clients.
• Serve as the primary point of contact for families and legally responsible parties, answering questions, providing updates, and addressing concerns.
• Handle other related duties as needed, contributing to the continuous improvement of services provided.
Experience/Skills
• A Bachelor’s Degree (B.A. or B.S.) in a related field, meeting applicable regulatory requirements.
• One year of experience working directly with individuals with intellectual and developmental disabilities.
• Strong communication skills, both verbal and written, for interacting with clients, families, and team members.
• Excellent organizational and time management skills to efficiently prioritize and manage multiple responsibilities.
• Decision-making abilities grounded in sound judgment, with a calm demeanor in challenging situations.
• Basic tech skills for completing documentation and managing email effectively.
• Willingness to learn about Medicaid, ICF/IID regulations, and licensure standards.
Requirements
• You must be 21 years of age or older.
• A valid Louisiana driver’s license and current automotive insurance are required.
• A satisfactory Motor Vehicle Record is necessary.
• You must be able to pass a background check, pre-employment drug test, and other related screenings.
• You must be available for weekend, evening, and late-night shifts as well as overtime when necessary.
