Job Description

Salary Range: $33,500 - $34,500
Our Support Coordinators (case managers) who serve the OAAS (Office of Aging and Adults Services) population, serve a caseload of about 35-45 participants. These participants live in their homes and communities and receive support (which can be physical, medical, educational, or vocational) to assist them to remain in their desired settings. Support Coordinators are responsible for the support of participants of daily living activities and throughout emergent situations including all public emergencies including weather related evacuations. While this position is a hybrid office and remote work, for the first 90-day you will be required to be in the office full time. This role requires providing supports in a home and community-based setting, so it is vital that you have a reliable form of transportation to visit participants in their homes.
Support Coordinator Qualifications:
- Bachelor’s degree or master’s degree in social work from a program accredited by the Council on Social Work Education; or
- Currently licensed registered nurse (RN); or
- Bachelor’s or master’s degree in a human service related field which includes psychology education, counseling, social services, sociology, philosophy, family and consumer sciences, criminal justice, rehabilitation services, child development, substance abuse, gerontology, and vocational rehabilitation; or
- Bachelor’s degree in liberal arts or general studies with a concentration of at least 16 hours in one of the fields listed in accordance with §10505.B.3.
- Reliable transportation so that all you to make visits participant homes. You may anticipate about 50% or more of your time will be in the field on home visits.
- The employee must be able to operate a computer and use web-based applications.
- Easterseals Louisiana is a Drug-Free Workplace, pre-employment and random drug tests will be administered.
- Will be required to pass Easterseals Louisiana’s background check and driver’s license check
- It is important to support coordination that employees understand that, barring accommodation, their commitment to our participant, and the waiver program group includes participation in office schedules and good attendance.
- The employee must be able to operate a computer and use web-based applications.
