Job Description
Direct Support Workers will be responsible for maintaining and enhancing the well being and health of individuals with developmental/ intellectual disabilities and the elderly. Primary goal is to help individuals reach individualized goals to maintain a level of independence in their community. DSW will assist with addressing ADL needs, household supports, meal preparations, accompany to medical appointments, help establish and maintain community interactions, etc. The DSW will be trained on client specific needs to help address individualized goals. DSW is required to utilize LaSRS to document time worked with individual and to complete daily progress notes to document activities accomplished through out the day.
