Job Description

The Director of Health & Wellness will serve as the clinical and programmatic leader for Split Second Foundation’s Comprehensive Outpatient Rehabilitation Facility (CORF) and its integration with an entrepreneurial wellness ecosystem that includes adaptive fitness, nutrition, resource navigation, case management, and financial planning support.

This is a unique opportunity to build a next-generation rehabilitation and wellness platform that aligns clinical care with holistic services, social determinants of health, and long-term independence. The ideal candidate brings both clinical rigor and entrepreneurial instincts—comfortable operating in outpatient healthcare environments while also innovating within a community-based nonprofit model.


Reporting Structure:

• Reports directly to the Executive Director

• Serves as the clinical leader across health and wellness functions

• Collaborates with fitness, operations, partnerships, and community engagement teams

Key Responsibilities:

• Stand up and oversee CORF operations including compliance, licensure, clinical standards, and interdisciplinary workflows.

• Integrate therapeutic interventions with wellness programs (fitness, nutrition, adaptive training, and community education).

• Lead care navigation supports including case management, resource referrals, insurance navigation, and discharge planning support for patients transitioning from acute care.

• Develop educational offerings such as financial literacy, special needs planning, and insurance education in partnership with financial advisors and community experts.

• Recruit, supervise, and support multidisciplinary staff (therapists, trainers, case managers, and wellness personnel).

• Establish performance, quality, and outcomes measurement systems for both clinical impact and client satisfaction.

• Cultivate strategic partnerships across health systems, rehabilitation networks, universities, social service agencies, and payors.

• Contribute to strategic planning, revenue development, and entrepreneurial growth initiatives.

Qualifications:

• Medical, clinical, or allied health degree required (MD, DO, DPT, OT, PT, RN, LCSW, or related).

• Experience in outpatient rehabilitation, integrated care, disability, or complex-needs populations strongly preferred.

• Knowledge of neurological and orthopedic conditions; exposure to spinal cord injury, stroke, TBI, MS, or aging populations valued.

• Excellent leadership, communication, and operational management skills.

• Entrepreneurial mindset with comfort building systems, teams, and partnerships in a growing organization.

• Mission alignment and commitment to disability inclusion and equitable access.

Performance Outcomes:

Success in this role will be measured by:

• Operationalization and licensure of CORF services within defined timelines

• Quality of care delivery and patient satisfaction

• Recruitment and management of a high-performing interdisciplinary team

• Development of data-driven outcomes and reporting frameworks

• Expansion of partnerships that enhance service offerings and access

• Financial sustainability benchmarks consistent with organizational strategy

• Retention and positive progression of clients across the care continuum
