Benefits or Limited Benefits

The RN Staff Nurse assesses, plans, implements and evaluates patient care based on the standards of care and the policies of LCMC Health. The RN delegates to professional and non-professional personnel involved in the delivery of care.

Limited Benefits

The following limitations apply to the benefits provided under this plan:

• Deductibles: A deductible must be met before benefits are payable.
• Co-payments: Certain services require a fixed co-payment amount.
• Co-insurance: After the deductible is met, the plan pays a percentage of covered expenses, and you are responsible for the remaining percentage.
• Maximum Out-of-Pocket Limit: Benefits cease once the annual out-of-pocket maximum is reached.
• Specific Service Limits:
• • Inpatient hospital days are limited to 45 per calendar year.
  • Outpatient physical therapy visits are limited to 20 per calendar year.
  • Substance abuse treatment is covered up to 14 days per lifetime.
• Exclusions: Experimental treatments, cosmetic surgery, and services deemed not medically necessary are not covered.
