Job Description
Linkage to Care Coordinator (1)
Acadiana CARES in conjunction with the Louisiana Office of Public Health-STI/HIV/Hepatitis Program is currently recruiting for an individual to be domiciled in Region 1 (New Orleans), to carry a caseload from both Regions 1 and Region 9 (Covington, Hammond, Northshore) to fulfill the following duties: 1) connect newly diagnosed persons with HIV (PWH) into medical care and community-based supportive services, 2) provide treatment adherence services to PWH who have fallen out of care to re-engage them with routine HIV medical care 3) provide treatment adherence services to PWH who have an elevated viral load, and 4)Make referrals to community resources to assist PWH overcome systemic and structural barriers.
Excellent skills in community engagement, facilitation, quality assurance and policy and procedure development. General knowledge of HIV, HCV, and/or STIs, case management, transmission, treatment adherence, health education and risk reduction.
Required: Qualified candidates must have a Bachelors in a social science or human related field. Two (2) years of social service experience working with vulnerable populations.
Preferred: Masters preferred with two (2) years of social service experience working with vulnerable populations.
