Job Description

Founded in 1935, Magnolia Community Services is a community of dedicated employees, families and medical professionals who are committed to helping the individuals we support to thrive, grow, and be recognized for their contributions to society.
We have an open position for a Case Manager. If you join our team as a Case Manager, you will have the privilege of leading and coaching our direct care staff who help our residents learn and grow each day in their homes and in the community.
As a Case Manager at Magnolia, you will ensure that individuals receive the support they need in all areas of daily living, which may include cooking, housekeeping, accessing the community, maintaining proper nutrition, exercising, and providing positive behavioral support and encouragement. You will work closely with our residents, their families and direct care staff to coordinate person-centered services for each resident on your case load. You will also serve as the liaison to the administrative team and outside professionals.
Requirements of the position:
· Bachelor’s degree in the human services field from an accredited university
· At least one year of experience working with people with intellectual/developmental disabilities
· Clear driving record with valid LA Drivers’ License and proof of insurance
· Clear Criminal background check
· Fully vaccinated for Covid-19
· Must be at least 21 years old
· Must be available to work a flexible schedule which may include Holidays and weekends and being on-call as needed.
· Must be available to evacuate with Magnolia in the event of a hurricane or other event requiring evacuation.
· Patience, compassion, reliability, flexibility
· Ability to communicate clearly and effectively both orally and in writing

Salary: $17.00 - $20.00 per hour
