Job Description
JOB SUMMARY
The Licensed Practical Nurse shall perform in accordance with Intermediate care facility ICF/DD Title XIX regulations and/or Home and Community Based Services (HCBS) within the guidelines of their professional license. All functions shall be performed in accordance with the established policies and practices of the institution in which an LPN is able to perform. The LPN is qualified by education, training, experience, and demonstrated abilities to give nursing care under the direction and approval of an RN.
Essential duties and responsibilities
To assess and monitor the health of the residents of the Volunteers of America SELA, Community Living Services Program; ensuring that quality outcomes are achieved, maintaining all documentation on individuals, reviewing individuals’ medical history and evaluating results consulting with medical professionals, observing staff, review medication administration and perform related duties as assigned.
Group Home & In-home
- Provided in-service training to staff as appropriate.
- Provide 24-hour on-call coverage as needed.
- Evacuate during hurricane season with the agency and resides on-site with resident and staff.
- Receive direction from RN.
- Perform related duties as assigned.
Functions
- Provide quality nursing care to consumers within the scope of the LPN license.
- Ensure consumers are treated with respect and dignity.
- Participate in consumer interdisciplinary team meetings, annual IHP/CPOC, and Quarterly updates
- Report incidents of neglect or abuse of consumers.
- Perform a complex task that can not be performed by DSP (i.e., wound care, shots)
- Provide support and advice for Med. Admin/CMA- certified medication attendants for tube feedings and medication administration if needed.
- Consult with physicians and other consultants to provide accurate and complete information concerning the current health and medical histories of consumers.
- Provide acute emergency protocols if needed.
Charting and Documentation
- Complete annually/quarterly health assessments for each consumer in Therap EHR.
- Maintain all required documentation; documentation is to be timely and accurate in accordance with the policy and procedures of the facility.
- Monthly report is to be turned in by the 5th of each month to RN.
- Perform routine charting duties as required. (i.e. discharge from hospital, admission to hospital, incidents with residents, change in behavior or diagnosis.)
- Accurately transcribes physician’s orders as per facility protocol and notifies RN, and other departments if necessary.
- Nursing notes are to be informative, and descriptive, and should reflect the care of the resident that has been provided.
- Review results from lab reports, medical evaluations, and physician notes and records recommendations in the consumer record book or Therap.
- Ensure that quality outcomes are achieved.
Personnel Function
- Attend nurse’s meeting as scheduled.
- Maintain a clean, safe, and efficient work environment.
- Maintain continuing education requirements.
- Make recommendations to the Home Manager/Facilitators based on observation of DSP performing health care procedures and provides an evaluation of their performance in medication passing.
- Develop and maintain a good working relationship with inter-departments, stakeholders, and consultant
Work Environment
- Expose to blood, body issue of fluids.

Knowledge skills and ability
Must have graduated from an accredited Licensed Practical Nursing program. Required to have at least two years of nursing experience and at least one year of nursing experience working with children and adults who have intellectual developmental disabilities, and related medical and physical disabilities. Must have excellent verbal and written communication skills.
Physical Requirements
Physical requirements include sitting, standing, climbing, pushing, bending, squatting, lifting, and pulling. May be required to assist in moving consumers weighing up to 200lbs. Other physical requirements include:
