CARE Team

At Care Eleven Health, our “Care Team” has a focus on community health, both for youth and adults. It is the mission of the team to provide accessible and convenient healthcare to the communities they serve. With an emphasis on early detection, wellness and prevention, the goal is to ensure that everyone in the community is healthy and thriving in all facets of life. Our services include: sports physicals, annual wellness visits, state mandated hearing and vision screenings, Telehealth services, education opportunities for students and staff in partner schools and workers compensation.

Care Eleven Academics is a network of school health clinics whose mission is to provide accessible healthcare to all students. With emphasis on early detection, wellness and prevention, our goal is to ensure all students, faculty and staff are healthy and thriving in all facets of life. Our services include: treatment of acute illnesses and injuries, COVID-19, cold and flu testing, immunizations and vaccinations, daily management of medications and chronic medical conditions, state mandated hearing and vision screenings, required sports physicals, telehealth services and educational seminars for faculty and staff
